This checklist helps people with chronic lower back pain build daily habits to reduce flare-ups, improve function, and know when to seek care. It’s for adults managing long-term back pain who want practical, low-risk self-care steps and escalation guidance.

  1. Schedule movement breaks every 30–60 minutes — Stand, walk, or gently stretch for 1–3 minutes each break
  2. Do a gentle core activation routine — Engage your lower belly to spine before activity
  3. Perform pelvic tilts — 10 slow reps — Lie on back, knees bent; tilt pelvis to flatten the lower back
  4. Practice bird-dog — 2 sets of 8 reps per side — From hands/knees, extend opposite arm and leg keeping a neutral spine
  5. Walk 10–20 minutes at a comfortable pace — Split into two short walks if needed to stay active
  6. Stretch hamstrings and hip flexors — 2×30s each side — Gentle static stretches to reduce posterior chain tightness
  7. Use heat or cold therapy appropriately — Heat for stiffness; cold for new swelling or post-activity flare
  8. Apply heat for 15–20 minutes when muscles feel tight
  9. Apply cold for 15–20 minutes after activity-related flare
  10. Optimize workstation ergonomics — Adjust chair, monitor at eye level, and keep feet flat on floor
  11. Use lumbar support or a rolled towel when seated — Support the natural curve to reduce strain while sitting
  12. Practice safe lifting: hinge at hips and brace core — Keep load close, avoid twisting, lift with legs not back
  13. Book physical therapy or specialist if not improving after 6 weeks — Consider PT, pain clinic, or spine specialist for persistent issues
  14. Use over-the-counter meds as directed or per your provider — Follow dosing instructions and check interactions with other meds
  15. Seek immediate care for red flags: severe weakness, numbness, loss of bladder/bowel, or fever — Call emergency services or go to the ER if any of these occur
