- Confirm patient identity and current ART regimen — Verify name, ID, and current medicines.
- Ask about missed doses and barriers to taking ART — Record any missed doses, reasons, and timing since last visit.
- Provide adherence counselling and issue ART refill — Give brief counselling, problem-solve barriers, and supply medicines.
- Check viral load result or collect sample if due — Review latest viral load; collect a sample if it's time per local schedule.
- Review CD4 count if indicated and order test if needed
- Screen for TB symptoms (cough, fever, night sweats, weight loss)
- Collect sputum or refer for TB testing when symptoms present — Arrange testing or urgent referral if TB symptoms are reported.
- Examine mouth for oral thrush and provide treatment or referral — Look for white patches and treat locally or refer for care.
- Ask about other new symptoms (skin problems, prolonged diarrhea, fever)
- Measure and record weight; compare to previous visit — Note weight change and flag rapid loss or gain.
- Offer brief nutritional counselling or refer for support — Give diet advice and refer to nutrition services if underweight.
- Screen for depression, anxiety, or substance use and refer if needed — Use simple questions and arrange mental health support when concerns arise.
- Ask about STI symptoms and offer testing or treatment
- Discuss family planning needs and offer pregnancy testing or contraception referral
- Review vaccination needs and arrange missing vaccines per local guidance
- Assess home and community support and refer to peer groups or services — Ask about support at home and link to community resources when needed.
- Plan the next clinic visit, document findings, and update records — Set follow-up timing and note actions in the patient's file.
